How To Purchase Your Plan

  1. Review Insurance Plans

    View full details and estimated cost of plans on https://www.ambadentalvision.com/trta.The Gold Dental Plan is great if you need basic coverage, while the platinum dental plan offers more comprehensive coverage. You can also add vision insurance separately or with your dental insurance.

  2. Go to Our Online Checkout Portal

    Select the dental and vision plans you want and add them to your cart. When you’re ready, click “Begin Checkout” to proceed. You’ll be securely redirected to AMBA’s online application portal to complete your plan enrollment.

  3. Fill Out Your Information

    The application process can be completed in as little as 5 minutes. You will be required to submit basic contact information for yourself and for your partner or eligible dependent children (if applicable). Once the application is complete, you can then submit your initial payment in our secure portal.

  4. Choose When Your Coverage Starts

    Coverage begins as early as the first of the following month, provided your initial premium contribution is paid within 31 days after the date you are billed. There is no waiting period for covered treatments once your plan becomes active. You can expect to receive your welcome letter in the mail from AMBA in a few days, which will include your unique plan, group, and online account information.

When does coverage begin?

How do I find out if my dentist is in network?

You can search for in-network dentists by using Ameritas’s Find a Provider tool and selecting Classic (PPO) & Plus from their drop down menu. Ameritas has an extensive network of dentists to choose from. You can also visit any licensed dentist, but you may save more by using an in-network provider.

Where can I use my vision benefits?

You can use your benefits with any provider, but you’ll save more by using one of VSP’s thousands of in-network providers, including Walmart, Costco, Visionworks, and Eyeconic. Find your eye doctor.

Participate in the Dental Rewards Program

Get more out of your dental insurance! Each year you receive dental care and keep your claims under a certain amount, unused benefits roll over and can be used in the future. Rollover up to $250/year in the Gold Plan or up to $350/year in the Platinum Plan within certain limits. With the dental rewards program, you have the flexibility to increase your maximum to $2,000 on the Gold Plan and to $2,750 on the Platinum Plan.
